Student-Athlete Services

We are excited to welcome your students to our athletic program for the upcoming school year. To ensure the health and safety of all participants, students must meet important requirements before they are eligible to participate in any school-sponsored sports activities. Rank One is the Electronic Medical Record system used to securely collect the required documentation.

Below is the information needed to make your athlete eligible for this season:

  1. Make a Rank One Parent account and claim your child’s account
    **If you cannot find your child in Rank One, please contact Candice to create their account**
  2. Fill out the 3 electronic forms required by NCISAA:
    1. Consent to Participate and Release Form: This is your acknowledgement and consent to allow your child to participate in athletics at Parrott Academy.
    2. Sudden Cardiac Death in Young Athletes Information: This is your acknowledgement about the risks, warning signs, and prevention of sudden cardiac death during sports activities.
    3. Concussion Acknowledgement Form: This is your acknowledgment that there is the risk of concussion in athletics and to report any signs/symptoms if you see them.
  3. Fill out the new Baseline Concussion Symptom Form: This questionnaire will provide cognitive baseline information on your athlete to retest in the event of a head injury. This form can be filled out through the Rank One app.
  4. Upload NCISAA Pre-participation Physical Evaluation: This packet is 4-pages long and includes a 2-page Health History to be signed by you and your child, as well as a 2-page physical portion to be signed off by a medical professional. This packet is eligible for 13 months. This form can be uploaded to Rank One via photos or PDF version. Please follow the link below for a PDF copy of the physical

  • All forms should be signed, uploaded and completed one week prior to the first practice.
  • PLEASE DO NOT turn in any HARD copies to your coaches (but keep them for your records)– everything should be completed electronically.

Rank One Instructions

Rank One is the Electronic Medical Records (EMR) system that APA’s athletic department has adopted to securely collect forms related to athlete eligibility and documentation of athletic injuries/illnesses. It is a REQUIREMENT of student-athletes to successfully set up an account and complete the necessary forms to participate in athletics. Here are the Rank One account set-up instructions:

Parents:
  1. Download Rank One Parent/Public app
  2. Select “I do not work at a school” and hit Continue
  3. Select “I am a Parent” and hit Continue
  4. Login in with existing account or Select “Create a New Account” and confirm email
  5. Return to App and login in with Account Login and Password
  6. Select “Add a Student”
    1. State: NC
    2. School District: Choose the school the student will be attending
    3. Enter student’s --> Last Name, First Name and Date of Birth
    4. Select 'Claim' 
      **Repeat Step 5 if you have multiple students
  7. Select Athlete(s) Name under “Your student(s)”
  8. Select “Forms” drop down
  9. Complete ALL forms (individually) under “Electronic documents to be submitted by the parent” 
Students:
  1. Download Rank One Parent/Public app
  2. Select “I do not work at a school” and hit Continue
  3. Select “I am a Student” and hit Continue
  4. Login in with existing account or Select “Create a New Account” and confirm email
  5. Return to App and login in with Account Login and Password
  6. Select “Add a Student”
